Capucine Holmes is from the Bay Area where she holds a Bachelors of Arts in Human Development Optioning in Childhood Education from California State University Hayward. She holds certifications in HIV/OSHA, CDC certified, Personal Outcome Measures training, Support Coordination training, ICG certified, and many other certifications. We are honored to have a self-motivated and gifted individual. Ms. Holmes assist providers’ with maintaining goal-oriented, strength-based, family driven services which address the client’s needs (such as, anger management, developing coping skills, etc.). Assist providers with developing individual and family oriented activities and curriculum that develops behavioral and emotional competencies. Prepare case management notes of supervision with providers weekly. Identify issues that are occurring, plan of action, interventions that are being used to assist providers with caseload; Maintain a small caseload and follow the policies and procedures under case management and documentation; Participate in the interdisciplinary team meetings to develop resources on behalf of the child and family; Ensure that all providers are maintaining documentation of daily services logs (time-tracking form) and submitting daily services logs in a timely fashion; Review all providers’ time sheets for adequacy and completeness; and many other duties.
